Patient Billing

Thank you for choosing Daniel H. Davis D.D.S. Our primary mission is to provide the patient with the highest quality dental care available in a warm, caring, anxiety free and professional atmosphere. We find much of the anxiety associated with dentistry comes with the uncertainty surrounding cost. An important part of our mission is making the cost of optimal care as easy and manageable on our patients as possible by offering several payment options.

Payment Options

You may choose from the following:

Cash, Check, Visa or MasterCard
We offer a 5% courtesy adjustment to patients who pay for their full treatment of $500 or more, with cash/check on the day of their treatment.
NO INTEREST 90 day Payment Plan through our office for those who qualify.
NO INTEREST Payment Plans for 4-12 months through City Health Card for those who qualify.
Convenient, extended low monthly payment plans available through City Health Card for those who qualify. (These plans are interest bearing accounts)

We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at (817) 656-9366. Many times, a simple telephone call will clear any misunderstandings.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.
